Time Lines and Service Requests

1. Submission of medical documentation   Student   At least two weeks prior to date of requested services
 
     
2.. Determination of student's eligibility for accommodations   Ross Center for Disability Services (RCDS)   Within two weeks of receiving documentation
     
3. Request for Disability Verification   Student, RCDS   Within two weeks of receiving documentation
     
4. Determination of specific accommodations for the individual student   RCDS   Within two weeks of receiving documentation
     
5. Meet with faculty to discuss specific services and get Confidential Faculty Letter signed   Student   Within three weeks after the beginning of classes
     
6. Completion of Exam Accommodation Form For regular exams / quizzes   Student, RCDS, Professor   Five business Days prior to the date of Final exams
     
7. Completion of Exam Form for final exams   Student, RCDS, Professor   At least two weeks prior to the date of Final exams
     
8. Request for alternative format for text book   Student   At least four weeks before before beginning of classes
     
9. Request for alternative Format for syllabi, handouts and exam material from RCDs   Student, Professor   At least two weeks before date needed
 
     
10.. Request for all other auxiliary services   Student   At least two weeks before date needed
