Eligibility of Service
I) What should
you do to become eligible for accommodations?
- Ask for Disability
Documentation Guidelines (Provided by the RCDS) as it pertains to your
disability.You can also visit the documentation page to view them online. Read the guidelines before the beginning of school. Gather appropriate
documentation for your disability from a licensed professional as it relates
to requested accommodations. You are responsible for any costs incurred in
obtaining documentation for your disability. Keep a copy of your documentation
for your own records. Be sure to contact us if you have any questions (617-287-7430).
- Schedule an
appointment with the appropriate Ross Center disability service provider.
Bring a copy of your documentation to your eligibility meeting.
II) What should
you expect during your first intake appointment?
- Complete a
pink Intake Form (located in our reception area).A review of
your documentation. A RCDS disability service provider will verify your eligibility
for accommodations and discuss what types of services and/or reasonable accommodations
you may need at this time. If the initial documentation is incomplete or inadequate
to determine the extent of the disability and appropriate accommodation, additional
documentation may be requested.
- Complete a
Semester Accommodation Form. Accommodations are determined
on an individual basis. It is the responsibility of the student to make an
appointment with the appropriate RCDS Service provider to discuss his/her
eligibility to receive accommodations. If it is determined that you are eligible
for specific services and/or reasonable accommodations, you must submit an
accurate and complete Semester Accommodation Form each semester. The purpose
for resubmission each semester is to assure that the RCDS staff has accurate
information for the timely delivery of accommodations, which may change each
semester depending on classes taken. Please refer to Requesting Accommodations
in the next section.
- A discussion
on the use of Confidential Faculty Letter for each of your professors.
The purpose of the letter is to verify that you have a disability and that
you are registered with the RCDS. It also tells your instructor(s) what types
of accommodations you are eligible to receive. You are responsible for meeting
with your professor/s to discuss the content of this letter. Both you and
your professor should sign it and you MUST return it to the RCDS within a
week after it has been signed. This letter will be available to you at least
one week prior to the start of classes.
