After it has been determined that you are eligible for academic
accommodations and wish to request an accommodation(s) you must
complete the Semester Accommodation Form and set
up an appointment to meet with the appropriate disability
service provider in order to receive services.
I)
What is the Semester Accommodation Form?
This form will provide the Ross Center for Disability
Services (RCDS) with the information necessary to coordinate
your academic services. You may pick up this form at the
reception area of the RCDS. If you need assistance with
filling out this form you will receive assistance from a
member of the RCDS staff.

When and how often should I complete this form?
A Semester Accommodation Form needs to be completed
at the beginning of each semester. If you are a new student
enrolled in classes you will complete one at the time of the
intake meeting. If you are a returning student, you will
need to complete one at the beginning of each semester or
when you are requesting new accommodations based on medical
changes.

What information do I need in order to complete this form?
You should have the following information when you complete
this form: 1) course titles, 2) course number(s), 3) meeting
day(s), time(s), and location(s) 4) name(s) of faculty. Once
you have completed the form, you should meet with the
appropriate disability RCDS specialist to review and discuss
your requests for that semester.
IV) How soon after completing the Semester Accommodation Form
will I receive services?
Once you complete the form, a disability specialist reviews
your request. If it is determined that you are eligible for
that specific accommodation, you will be given a
Confidential Faculty Letter. When you return the signed
Confidential Faculty Letter, your services will be
coordinated. There are different timelines based on the
service(s) you request.
The RCDS will make every effort to accommodate you, and you
should make every attempt to adhere to these guidelines. The
guidelines are being implemented to ensure that you are
receiving reasonable and appropriate accommodations.
